Post surgical pain treatment

Persistent Post-Surgical Pain (PPSP)

When pain continues after surgery, recovery doesn't stop—neither do we

Most people expect to feel some pain after surgery, but for some, that pain doesn't go away. Persistent post-surgical pain—also known as PPSP—is pain that lasts longer than 3 months after surgery, even though the surgical site has healed. It can be frustrating, confusing, and deeply impact your recovery and quality of life.

At Wellspring Pain Clinic, we recognise that ongoing pain after surgery is real, and treatable. Our focus is to help you understand what's happening, reduce pain safely, and support you in regaining your strength, confidence, and function—without long-term reliance on medication.

What Causes Persistent Post-Surgical Pain?

PPSP is often due to changes in the nervous system during or after surgery. Common contributing factors include:

  • Nerve injury or entrapment during surgery
  • Scar tissue affecting nearby nerves or structures
  • Inflammation or abnormal healing
  • Central sensitisation – when the nervous system becomes hypersensitive
  • Pre-existing chronic pain conditions
  • Inadequate early pain control or immobilisation

Surgeries commonly associated with PPSP include:

  • Spine surgery
  • Joint replacement (hip/knee/shoulder)
  • Hernia repair
  • Amputation (phantom limb pain)

What Does PPSP Feel Like?

  • Burning, stabbing, or shooting pain
  • Numbness, tingling, or altered sensation around the surgical site
  • Hypersensitivity to light touch or pressure
  • Pain that radiates to nearby areas (e.g., down the leg or arm)
  • Sleep disturbance, fatigue, and reduced function

How We Help at Wellspring Pain Clinic

We use a comprehensive, multidisciplinary approach to treat PPSP. Our aim is not just to "numb" the pain but to help you understand it, manage it, and move forward.

1

Thorough Assessment

We carefully review your surgical history, pain patterns, and any imaging or test results. We identify the likely source of your pain—whether it's nerve-related, joint-related, or sensitisation-driven.

2

Education and Reassurance

Understanding why the pain is happening helps reduce fear and anxiety. Studies show that patients who understand their condition are more likely to improve.

3

Physical Rehabilitation

We work with physiotherapists and exercise physiologists to:

  • Improve mobility around the surgical area
  • Rebuild strength and stability
  • Correct movement patterns
  • Reduce fear of re-injury or "causing more damage"
4

Psychology and Pain Coping Strategies

Chronic post-surgical pain often affects sleep, mood, and mental health. Our team may recommend pain psychology to help with:

  • Stress and emotional adjustment
  • Fear-avoidance and catastrophising
  • Sleep strategies and pacing
5

Interventional Procedures (if needed)

For some patients, targeted procedures can help break the pain cycle, including:

  • Pulsed radiofrequency (PRF) – calms irritated nerves without damaging them
  • Nerve blocks – for diagnostic or temporary relief
  • Scar tissue infiltration – if adhesions are contributing to pain
  • Peripheral nerve stimulation – for persistent neuropathic pain
  • Spinal cord stimulation – in complex, treatment-resistant cases

Helping You Reduce Reliance on Medications

Many patients with PPSP are placed on multiple medications, including opioids, nerve pain agents, or antidepressants. While medications may help short term, long-term use can lead to:

  • Tolerance or dependence
  • Side effects (e.g. drowsiness, brain fog, constipation)
  • Reduced motivation to re-engage in physical rehab

At Wellspring, we work to reduce or eliminate long-term medications wherever possible, replacing them with active, empowering strategies.

You're Not Alone in Your Recovery

Post-surgical pain that won't go away can feel isolating and overwhelming. But with the right support, understanding, and treatment, you can move forward. Whether your goal is to return to work, exercise again, or simply enjoy daily activities without constant pain, we're here to help you achieve it.
